Dr. Vitolo is an expert in performing transumbilical breast augmentation surgery (TUBA), a breast enlargement procedure that leaves no scars behind for a supremely natural appearance. In this procedure, Dr. Vitolo uses the body’s one natural scar, the belly button, to insert and position the implants. This method is minimally invasive and also allows for the implants to be individually sized, a useful benefit for women whose breasts are different sizes.

The most frequently performed cosmetic surgery procedure in the United States, breast enlargement can give women with small or unevenly sized breasts a fuller, firmer, better-proportioned look. Women may elect to undergo breast enlargement for many different medical and aesthetic motivations, including balancing breast size and compensating for reduced breast mass after pregnancy or surgery. In breast enlargement surgery, breast implants are placed behind the patients’ natural breasts, either underneath the breast tissue or underneath the chest muscle. Breast enlargement may be combined with another procedure, such as a breast lift, for more satisfying results. Women who elect to undergo breast enlargement surgery may choose implants made of silicone or saline. Saline implants are composed of a silicone shell filled with saline (salt water). Silicone gel implants are composed of silicone, and their texture has earned them the nickname “gummi bear implants.” Our practice uses the Natrelle line of silicone gel filled implants, manufactured by Allergan. A breast lift, or mastopexy, is performed to return youthful shape and lift to breasts that have begun to sag as a result of weight loss, pregnancy, loss of the skin's natural elasticity, or simply the effects of gravity. The procedure can also reduce the size of the areolas (the dark skin surrounding the nipple), and can be combined with breast enlargement for added volume and firmness. Breasts of any size can be lifted, but results last longest with smaller breasts. Mastopexy may be performed in a hospital, an outpatient surgery center, or a surgeon's office-based facility. It is usually performed under general anesthesia and lasts from one to three hours.
Large breasts may cause physical and mental discomfort and can result in significant health problems for the women who have them. The size and weight of large breasts can result in self-consciousness, improper posture, pain in the back and neck, indentations from bra straps, skin rashes, breathing problems, and skeletal deformities. At our New York City area practice, breast reduction surgery is usually performed to provide physical relief from these symptoms. During surgery, the patient is under general anesthesia. The process lasts from two to four hours, during which time Dr. Vitolo removes fat and glandular tissue and trims resultant excess skin to produce smaller, lighter breasts that are in a healthier proportion to the rest of the body. Breast reduction surgery is not recommended for women who intend to breastfeed, since many of the milk ducts leading to the nipples are removed in the process of reducing the size of the breasts. Modern surgical technology makes it possible to construct a natural-looking breast after mastectomy (breast removal) for cancer or other diseases. The procedure is commonly begun and sometimes completed immediately following mastectomy so that the patient wakes with a new breast mound. Alternatively, reconstruction may begin years after mastectomy.
There are several ways to reconstruct the breast, both with and without implants. Dr. Vitolo will work together with you to help you decide which option suits you best. Breast reconstruction is not associated with the recurrence of cancer or other diseases, or the disruption of chemotherapy or radiation treatment.
Although it is rarely discussed, gynecomastia, or enlarged male breasts, is a common condition affecting approximately 40 to 60 percent of men. There is no known cause for gynecomastia, although researchers have established several correlations with various drugs or medical conditions.
Men who feel self-conscious about their appearance can be helped with gynecomastia treatment, a surgical treatment that removes fat and/or glandular tissue from the breasts. If needed, excess skin may also be removed to achieve a contoured chest that is flatter and firmer. Male plastic surgery is becoming increasingly common as more men discover the procedures available to them.
